The specimen shows a normal mitral valve below (M), with a rectangular defect presumably made at the time autopsy or specimen mounting) and what appears to be a bicuspid aortic valve above. The aortic leaflet on the left is almost completely destroyed by endocarditis, which has extended into the adjacent endocardium (arrow (A)). Further extension of the infection through the myocardium has resulted in small aneurysmal dilatations near the anterior leaflet of the mitral valve (arrows on back view (B)).

Comment

The descriptive card refers to case MGH 476.76. However, no case history or autopsy findings corresponding to the specimen can be found in Osler's autopsy books.
