Premise: You were born in Quebec
One of the following documents is required:
- QC birth certificate or copy of an act of birth: issued by the, and must feature the notation Certifié conforme or Certified. The certificate must have been issued after January 1994. Certificates marked Certifié conforme à l'article 137 are only acceptable to prove Canadian status, and not Quebec residency.ÌýStatements of live birth that have been noted with the following are not acceptable: "This document is not a birth certificate" or "this document is not an act of birth".
- Other document issued by the Directeur de L'État Civil, where the place of birth is a clearly identifiable location in Quebec, e.g., a marriage certificate.
- Canadian passport: A valid or expired Canadian passport is acceptable where the place of birth indicated is a clearly identifiable location in Quebec AND you already have a Quebec permanent code which has been validated to us by the Ministry of Education (i.e. the perm code is visible in your Minerva account at Student>Student Accounts>View Tuition & Legal Status).
N.B. Baptismal certificates are no longer acceptedÌýby the Ministry as proof of Quebec residency.
